Backport PR #5584: more semantic icons This adds classes to customize the folder and notebook document icons in the dashboard view. I've also changed the notebook icon to have it better distinguished from the folders and match e.g. Github's approach to this issue. For the pixel-perfectionist, I've also made these icons a fixed width (they weren't before, I'm sure, like me, it's made you angry ; ) screenshot is coming. closes #5555

depend on [CasperJS](http://casperjs.org/), which in turn requires a recent
version of [PhantomJS](http://phantomjs.org/).

r15262 The file `util.js` contains utility functions for tests, including a path to
a running notebook server on localhost (http://127.0.0.1) with the port
number specified as a command line argument to the test suite. Port 8888 is
used if `--port=` is not specified. When you run these tests using `iptest`
you do not, however, have to start a notebook server yourself; that is done
automatically.
