incremental improvements to SSH launchers * add to_send/fetch steps for moving connection files around. * add SSHProxyEngineSetLauncher, corresponding to `ipcluster engines` on a remote host. This can be used to start a set of engines via PBS/SGE/MPI *remotely*.
